Anking/Miledown are smaller decks but still very good. if you have a lot of time and want to cover every single piece of material, use JS.

With separating the decks, i haven't had that problem as each one functions on its own. Only thing that will still affect it is let's say you increase the number of new cards for the individual decks, each deck, even tho they are in their own separate deck, will change. That's universal for Anki tho I think. ie: I have 50 new card and 100 ...It’s the only anki I used and I turned out fine. They have a deck on anki too called the miledown anki deck. download it and use both and your content review should be pretty good as long as you did alright in pre-reqs.

You really need to be taking practice tests and use the AAMC QPACKS and SBs. However, if you're asking if for the resource of ANKI alone, if you only use one deck you'll be fine. 99% of people I know only use one deck. But they add/take away from the deck as needed.
