It doesn't matter, so just pick what you think is the most accurate. It sounds like you feel like the first one should be rural and the second one should be urban. Ultimately, it doesn't matter. If you get asked about it, it sounds like you can defend either decision rather easily. I seriously don’t think they really care, i’ll be honest ...I did a month-long derm rotation(am a EM resident) during Covid and saw a lot of telemedicine and the idea of doing a telemedicine only practice in Derm seems like taking all the worst parts of Derm and doing that all day long.I've spoken to several people in my geographic area and most people seem to be leaning towards advising students to take a research year. Obviously the advice depends on an individual's application, but with stuff being P/F now, I think advisors are concerned papers and letters will matter even more hence increased recommendation for RY.
The reality is that there are dermatologists who become mohs surgeons, nobody reasonable is going to pretend like having an inclination towards that is nonexistent among applicants. Yes it does look bad. Most medical/academic derms would prefer an interest in general / medical derm.
